Plain-English Summary

This rule prints the notice a party files to appeal a compulsory-arbitration award. Addressed to the prothonotary, it gives notice of the appeal and certifies either that the arbitrators’ compensation has been paid or that the party has applied for permission to proceed without paying costs. No affidavit or verification is required.

The note adds that the demand for a jury trial on the appeal is governed by a separate rule. The simple form and the fee certification are the gatekeepers that move a case from arbitration to a de novo trial.
