Plain-English Summary
Section 6-1002 does the heavy lifting for Article 10: it adopts the form. Under authority granted by Neb. Rev. Stat. § 25-1657, the Nebraska Supreme Court has adopted the Nebraska Juror Qualification Form, attached to the rule as Appendix A, and every district and county court must use it whenever a statute requires a juror qualification form — unless the court has gotten Supreme Court approval for an amended version through the process described in § 1-103.
The rule carries a built-in transition. Through December 31, 2025, courts use the paper form as adopted. After that date, the requirement expands: courts must keep using the approved form, but must also enter the information it collects into the e-Jury system through JUSTICE, or into independent jury management software the Supreme Court has separately approved, so that juror data is gathered and kept electronically rather than only on paper. Any request to amend the form itself, or to adopt different jury management software, still goes through § 1-103.
