New Jersey Divorce Forms: What to File, Form Numbers, and Where to Get Them

New Jersey family law · Last reviewed October 23, 2026

New Jersey's Judiciary numbers its forms with a CN ("catalog number") and posts them in its online Forms Catalog. The core divorce papers, the complaint and its certifications, don't have a statewide CN. You draft them, or get them from a kit: "Legal Services of New Jersey (LSNJ)" offers a free guide and "They also sell a complete divorce kit (forms + instructions) for $25." The local ombudsman office at your courthouse can also give you the forms and explain the steps, though staff can't give legal advice.

Filing the case

The Judiciary's checklist for starting a divorce:

Family Part Case Information Statement1048208/2025 (eff. 09/01/2025)The financial disclosure; required when custody, support, alimony or property is disputed

The descriptive material on alternatives to litigation is CN 10888, "Dispute Resolution Alternatives to Conventional Litigation." The rule behind the insurance and confidential-information papers is Rule 5:4-2.

Filing tips from the Judiciary: in JEDS, the online system, "upload each document separately. Do not combine them into one file." In person, "Bring three copies of your papers to the Superior Court Family Division in your county." By mail, send two copies and keep one, with a stamped, self-addressed envelope.

Responding

A defendant files an Answer, an Answer and Counterclaim, or an Appearance, within 35 days after service. The Judiciary's responding page lists the same certifications: insurance coverage, CN 10486, CN 10889, redaction of personal identifiers, and verification and non-collusion. The Case Information Statement (CN 10482) follows within 20 days after the Answer or Appearance.
