Nevada Divorce Forms: Joint Petition, Complaint, Resident Witness Affidavit and Decree
Nevada family law · Last reviewed October 27, 2026
Nevada has no single mandatory form set. The courts' statewide self-help center publishes forms and guided interviews, and the two big counties publish their own: Clark County through its Family Law Self-Help Center, and Washoe County through the Resource Center of the Second Judicial District Court. Use the forms for the county where you'll file.
Filing together: the joint petition
| Form | What it does |
|---|---|
| Family court cover sheet | Opens the case for the clerk |
| Confidential information sheet (with or without children) | Gives both Social Security numbers, which the court keeps confidential |
| Affidavit of resident witness | A third person's sworn statement that one spouse has lived in Nevada six weeks |
| Joint petition for divorce (with or without children) | Your full agreement, signed under oath by both spouses |
| Decree of divorce for a joint petition | The order the judge signs |
In Clark County, both spouses sign the joint petition before a notary and complete the decree before the judge signs. Washoe County's packets are D-2 (no minor children) and D-4 (with minor children). The statewide set has the same parts.
Filing alone: the complaint
| Form | What it does |
|---|---|
| Family court cover sheet | Opens the case |
| Summons | Tells your spouse about the case and the 21-day deadline |
| Complaint for divorce (with or without children) | States residency, grounds and what you ask for |
| Affidavit of service | Proof that your spouse was served |
| Waiver of service, or alternative service and publication forms | If your spouse waives service or can't be found |
Washoe's packets are D-6 (complaint, no minor children) and D-7 (complaint, with minor children).
Responding
The defendant files an answer, or an answer and counterclaim that says what he or she wants. Washoe numbers these AC-1 and AC-2. Mail a copy to your spouse and file a certificate of mailing.
Financial disclosure
Each spouse files a General Financial Disclosure Form within 30 days of service of the summons and complaint (NRCP 16.2). The state self-help center tells defendants to file it within 30 days of filing the answer, but the rule counts from service, so file it early. Spouses with high incomes, large assets or a business may need the detailed form instead.
Finishing the case
To get a decree without a hearing in Clark County after a default or settlement, file a request for summary disposition, the affidavit in support, the affidavit of resident witness and the confidential information sheet, then send the proposed decree of divorce to your judge. After the decree is filed, file and serve a notice of entry of order. If you started with a complaint and later agree, Washoe's C-1 and C-2 convert it into a joint petition.
Guided interviews
Both the statewide site and Clark's center offer question-and-answer interviews that fill in the forms. Clark's joint petition interview is called Guide and File. Clark tells e-filers to upload each form as its own PDF, because one combined file "will significantly delay the processing."
A few cautions
- Match the decree to your route. Clark warns that complaint-case decrees are not for joint petitions, and the reverse.
- Visitation must be specific. An order must define visitation "with sufficient particularity" and include the specific times (NRS 125C.010).
- Name change. Say in the petition or complaint if you want a former name restored.
- Separate maintenance and annulment have their own forms.
