Arizona Divorce Forms: Petition, Response, Consent Decree, Default and Parenting Plan

Arizona family law · Last reviewed November 2, 2026

The Arizona Supreme Court's Self-Service Center publishes divorce forms that, in its words, "are generic and may be accepted by courts statewide." Each court might prefer its own, so check your county's self-help center (in Maricopa, the Superior Court's Law Library Resource Center) before you file. The statewide packets are for non-covenant marriages; if you have a covenant marriage, ask the court for the right forms.

Fill in forms carefully: a petition is verified, which means you sign it under oath (§ 25-314).

Frequently asked questions

Where do I get divorce forms in Arizona?

From the Arizona Supreme Court's Self-Service Center (Family Law Forms) or your county superior court's self-help center. The statewide forms may be accepted by courts across the state, but some courts prefer their own.

Are there forms for a covenant marriage divorce?

The statewide packets are titled for non-covenant marriages. A covenant divorce needs a § 25-903 ground stated in the petition, so ask the court's self-help center which forms to use.

The Petition and Response for Summary Consent Decree, form DRDSC11F, signed by both spouses and filed with the preliminary injunction copies. The proposed decree (DR71F) and your agreements follow within 60 days.

Do I need a parenting plan?

Yes, if you have minor children in common. A consent decree must include legal decision-making and parenting time provisions, either in the decree or in a separate parenting plan.

What is the sensitive data cover sheet?

A cover sheet the forms label a confidential record, filed with the petition and with the response. There are two versions, one for cases with children (DRSDS10F-C) and one without (DRSDS10F-A). Your court's self-help center can explain what goes on it.
