Notes

Advisory Committee Note — 2019 Amendment: The amendments delete the general-practice forms previously appended to the NRCP. In their place, the introduction to Appendix of Forms lists some of the on-line, self-help, and other resources available to practitioners and self-represented parties. As amended, the Appendix of Forms includes forms addressing waiver of service under Rule 4.1, consent to electronic service, and financial disclosures in family court. The elimination of the general-practice forms does not alter existing pleading standards or otherwise change the requirements of Rule 8.

Plain-English Summary

Rule 84 gives official standing to the forms gathered in the NRCP's Appendix of Forms. Instead of the general-purpose pleading forms once attached to the rules, the appendix now focuses on a handful of specific, commonly needed documents — including a waiver-of-service form tied to Rule 4.1, a form for consenting to electronic service, and financial disclosure forms used in family court.

The appendix also points litigants and self-represented parties toward online and self-help resources beyond the forms themselves. Dropping the older general-practice forms doesn't change what a pleading has to contain — Rule 8's pleading standards still apply regardless of whether a party uses a form from the appendix or drafts its own document.

Frequently Asked Questions

Are the forms in the Appendix of Forms mandatory?

Rule 84 authorizes their use; it doesn't require every filing to use a form instead of a custom-drafted document.

What kinds of forms are in the current Appendix of Forms?

It includes a waiver-of-service form connected to Rule 4.1, a consent-to-electronic-service form, and financial disclosure forms for family court, along with pointers to other self-help resources.

Where did the old general-practice pleading forms go?

They were removed from the appendix; self-represented parties are directed instead to online and self-help resources for general pleading guidance.

Does using a form from the appendix guarantee my pleading meets the rules?

No. Using an authorized form doesn't override Rule 8's pleading standards — the content still has to satisfy what Rule 8 requires.

Can I use my own version of a document instead of the appendix form?

Rule 84 authorizes the appendix forms for use; it doesn't bar a party from filing a properly drafted document that isn't built on one of them.
