(a)The written notice of intent to serve a subpoena required by Rule 4009.21(a) shall be substantially in the following form: (CAPTION) NOTICE OF INTENT TO SERVE A SUBPOENA TO PRODUCE DOCUMENTS AND THINGS FOR DISCOVERY PURSUANT TO RULE 4009.21
